Description

Health Horizons International (HHI) is a non-profit organization that strengthens community health and promotes high quality primary health care in impoverished communities on the north coast of the Dominican Republic. We support a comprehensive vision that addresses the root of health problems in partnership with the region's most vulnerable people. Our model includes:

  • Building Health Care Capacity -- a community health workers program focusing on patient care, particularly chronic disease management, and health education
  • Dignifying Patient Care -- primary health care field clinics every four months to provide underserved patients with continuity of care and access to a network of sub-specialists
  • Promoting Public Health -- a commitment to public health research and collaboration to address the underlying determinants of health and illness and to strengthen the local health system
